$150 every 4 week cycle (established patients, on discounted rates)

  - For established patients, during third month, if no missed appointments / late payments

  - If requested by patient, due to financial hardships

  - Does NOT apply to patients on medications for multiple disorders requiring controlled substance.

  - This will cover in-office & Tele visits with physician / provider and in-office drug screens as well.

  - Discounted rate will not apply if more than 45 days have passed since last visit

$160 every 4 week cycle (established patients)

$175 every 4 week cycle (NEW patients)

  - If paid via cash or Zelle or Chase quickpay (contact@vitaloptions.com)

  - $180 every 4 weeks via credit or debit card

  - This will cover in-office & Tele visits with physician / provider and in-office drug screens as well.

 

There is no cancellation fee because stuff happens, however, in case of multiple cancellation requests without a valid reason, we might have to terminate services.

 

Please note that previously our charges for first month or induction, were $250 per month. We listened to our patients request and as of Aug 2019, charges for even the first month will be flat $175 per month, which includes drug screen(s) as well.
